Accessing the Services
You may view and print our application by clicking on Application Form for Individuals as a Word Document or Application Form for Individuals in Adobe PDF Format. Since we are required to maintain a file of original certifying signatures, please print out the application, have it signed, stamped, and mail it to us.
If you are applying to start service for an institution such as a school or nursing home, please fill out the Application for Free Library Service - Institutions as a Word Document, have it certified and signed by the administrator, and mail it to the Staunton Talking Book Center. You will receive a confirming phone call as soon as we receive your application.
